Program Description & BenefitsHelping participants identify unmet health needs

In this free program, offered in partnership with DHHS, trained Linkage Coordinators help individuals with IDD, TBI, and mobility limitations, unmet health needs, and connect with the right resources and support services.

Based on each participant’s needs and goals, coordinators provide referrals to healthcare providers, mental health providers, health promotion programs, and other community services.

Participants in the program may experience:

Increased access to preventive and mental healthcare

Improved health and wellness behaviors

Better management of chronic conditions and health risk factors

What Participants ReceiveContinual screening for next steps

Participants meet with a Linkage Coordinator at a screening appointment to determine unmet needs. The participant then receives a referral to a preventative healthcare provider, mental healthcare provider, health promotion programs, or other services, such as transportation or housing, based on their answers to the screening questions. Participants will have a 3-month follow-up appointment to ensure needs have been met and to receive new referrals if needed.

At the screening appointment and 3-month follow-up participant will receive:

Knowledge assessment

Health education

Referrals & resource connections, as needed

Rules for Participation Eligibility requirements

To participate, individuals must:

Be 18 years or older

Be a Utah resident

Have an intellectual or developmental disability, traumatic brain injury, or mobility limitation

The survey will take approximately 8 minutes to complete. This form will be used to refer people with intellectual and developmental disabilities to the Linkage Coordinators at Utah Health Policy Project.  Please fill out as much information as you can.  The Linkage Coordinator will contact the participant to schedule the screening appointment.  If you have any questions, please email the Linkage Coordinators at linkage@healthpolicyproject.org.
