Liability & Consent

I understand that volunteering may involve standing, walking, lifting light materials, and interacting with the public. I agree to follow all event instructions and safety guidelines.

I, the above listed Volunteer, desire to work as a volunteer for Utah Health Policy Project (UHPP) and engage in the activities related to being a volunteer for a work event. 

Volunteers understand that:

  1. The scope of a Volunteer’s relationship with UHPP is limited to a volunteer position, and no compensation is expected in return for services provided by Volunteer
  2. Utah Health Policy Project will not provide any benefits traditionally associated with employment to a Volunteer
  3. The Volunteer is responsible for their insurance coverage in the event of personal injury or illness due to volunteer services to Utah Health Policy Project
  4. In consideration for being allowed to participate in this activity, the Volunteer releases from liability and waives their right to sue Utah Health Policy Project, their employees, officers, volunteers and agents from any and all claims, including claims of Utah Health Policy Project's negligence, resulting in any physical injury, illness (including death) or economic loss the Volunteer may suffer or which may result from their participation in this Activity, travel to and from the Activity, or any events incidental to this Activity
  5. By signing this waiver, the Volunteer grants Utah Health Policy Project the right, title, and interest in any photographs, images, video, or audio recordings of them or their likeness or voice made by UHPP in connection with providing volunteer services to UHPP
